Class, Lecture or Workshop On-Line Form.

Keep in mind that all classes are subject to board approval.  CREATE requires a 30% fee from any instructors requesting payment.   Should the instructor be willing to share their knowledge free of charge, we will request a donation of any amount  from attending students.   Please do not forget to include costs of supplies.
